Pieczęcie " Z biblioteki Seminarium Sandomierskiego". Karta tytułowa na obwolucie bardzo ozdobna i kolorowa, na następnej karcie jest druga strona tytułowa, z bardzo zbliżonym tekstem, już bez ozdób. Obie były pierwotnie zszyte zieloną nitką, becnie zszycie tylko w części grzbietowej. Na owej wewnętrznej karcie tytułowej jest daa dzienna: Leopoli die 13 Junii, oraz tytuł arii "De S. Matre Anna". Na głosie vl3 : "Violino tertio. Viola transposita" - widocznie pierwotnie partia ta była przeznaczone na altówkę.
